Investigation Summary
At 6:30 a.m. on November 1, 2019, an employee was working for a big box retailer of home improvement supplies. He was standing on a rolling ladder to access shelves for upstocking and downstocking inventory. He fell from the rolling ladder and struck his head on the floor. He sustained a fractured his skull, and he was bleeding from his head. The employee was hospitalized. He died at the hospital.
