Investigation Summary
At 7:00 p.m. on November 2, 2017, an employee was operating a motorized stand-up pallet jack in a warehouse setting. The employee was driving an aisle in a food distribution warehouse. The employee crashed into a set of shelving units that were adjacent to the aisle way. The employee was pinned between the pallet jack and the shelving units. The incident resulted in fatal injuries to the employee. Medical Examiners report determined that cause of incident was due to medical condition/event which caused the employee to lose control of pallet jack.
