Investigation Summary
At 5:10 p.m. on August 20, 2019, Employee #1 and a coworker, employed by a construction company, were working at a multiemployer construction project. They were working from inside a platform debris-box, lifted on the forks of a forklift, when the debris-box tipped over and fell. Employee #1 and coworker fell to the concrete surface, a fall height greater than 6 feet. Employee #1, who was struck by the falling debris-box, sustained severe head trauma and was killed.
