Investigation Summary
At 2:15 p.m. on January 28, 2019, an employer was providing instruction on where drywall needed to be placed. Employee #1 was looking up and leaned forward, and fell 22 feet below landing on scaffold. The employee sustained fractures, lacerations, internal bleeding and died.
