Investigation Summary
At 4:00 p.m. on November 25, 2018, an employee was standing on an inoperable forklift. Another employee turned and noticed the employee standing on the forklift and motioned and yelled at him to get down. He then turned back to his work, and when he looked back at the forklift, he saw the employee's head caught in the forklift mast. The employee died from a skull fracture with laceration of the forehead and brain from being struck-by and caught-in the forklift mast.
