Investigation Summary
At 12:00 a.m. on May 27, 2020, an employee was performing a routine oil change on a Chevrolet 3500 box truck. He drove the vehicle onto a four-post, surface-mounted vehicle lift; the lift was rated for 12,000-pound capacity. He lifted the vehicle approximately two feet off the ground and went under the vehicle to begin draining the oil from the truck's engine. The total weight raised and being held by the lift was 12,170 pounds. The first position of the lift's safety brackets were 33 inches from the floor level. The height of the truck and dimensions of the service bay prohibited the lift from being raised high enough to engage the first safety brackets. The lifting mechanism failed and, without the safeties engaged, the lift and vehicle fell to the floor, trapping the employee between the underside of the vehicle and the concrete floor. The employee was killed as a result of blunt traumatic injuries to the chest and abdomen.
