Investigation Summary
At 3:45 p.m. on December 4, 2017, an employee was climbing a scaffold ladder and accessing the scaffold platform. The employee grabbed onto the guardrail to climb underneath it when the rail came undone and the employee fell backwards from the scaffold. The employee sustained a skull fracture and was killed.
