Investigation Summary
At 2:00 p.m. on April 22, 2017, an employee was driving a cart east on the dam from the Keokuk Power Plant (Iowa) to the Hamilton Sub Station (Illinois). While driving, the cart veered north and crossed over a set of rail road tracks, a compressed air line, and a rail for a gantry crane. The cart and the employee fell approximately 11 feet from the dam and into the upstream side of the Missippi River between gates 110 and 111. The employee died as a result of drowning.
