Investigation Summary
At 1:30 p.m. on October 23, 2018, an employee was moving roofing material from one place to another on an elevated flat roof using an ATV with a trailer hitch when he drove off of the roof. The employee was ejected from the ATV and fell approximately 40 feet before landing on the pavement. The employee was killed.
