Employee is killed when struck by valve

At 2:00 p.m. on May 31, 2020, Employee # 1 was working on the well, servicing the rig floor. The high pressure line coming off the manifold to the tubing was over pressurized causing the two inch valve to strike the employee. He suffered blunt force trauma to the head and was killed.
