Investigation Summary
At 9:55 a.m. on June 27, 2018, an employee was making preparation to mow an orchard using a 1992, John Deere, orchard low-profile tractor (model number: 2755). The tractor had a Rhino center pull brush-hog type mower attachment from the early 1990s (model number: TW 120) affixed to it. As the employee started the tractor, which was already in gear, it started moving on its own. The tractor travelled approximately 90 feet and up onto an embankment and ran over the employee who made attempts to stop it. The employee suffered lacerations/fractures to his lower legs and pelvis region, resulting in his death.
