Investigation Summary
At 3:30 p.m. on July 18, 2018, an employee and coworker were installing a laundry center in a supply unit. The employee attached a 30-amp, 4-prong wire to the laundry center. While connecting the laundry center to the electrical outlet, the employee stiffened, collapsed, and was unresponsive. The coworker pulled him out of the closet and summoned an ambulance. The coworker returned to the employee and administered chest compressions until medical assistance arrived. The employee was transported to a hospital. The employee died.
