Employee is killed when pinned beneath a forklift

On February 21, 2018, an employee was using a forklift to stack empty pallets. As the employee raised the forks of the forklift to its highest point to place a pallet at the top of the pile, the forklift tipped forward. the employee became pinned beneath the forklift. He suffered crushing injuries and was killed.
