Investigation Summary
At 3:00 p.m. on September 4, 2019, an employee was defueling vehicle gasoline tanks in an auto salvage yard when the gas vapors near the ground surface were ignited by torch cutting operations. The vapors ignited and the employee was killed in the explosion.
