Investigation Summary

At approximately 12:15 p.m. on July 19, 2018, an employee was troubleshooting a Trane Voyager 25-ton HVAC system on the east side of a newly constructed addition/expansion building project. The furnace portion of the unit was not functioning properly. The power was turned on and off several times during the morning. When the accident occurred, the power to the unit was on. The employee made contact with an electrical lead connected to a transformer inside the unit and was electrocuted. The electrical lead was connected to power going directly into the unit. A burn mark was located on the electrical lead. The unit carried a range of 414 to 506 volts and 54 to 70 amps. The control panel section, where the employee made contact with the electrical lead, was rated 480 volts nominal. The employee was working alone. The employee had a burn mark on his left finger. The unit was not locked out. The employee was not wearing a flame resistant shirt, electrical gloves, head liner, or arc flash face shield while troubleshooting the energized unit.
