Investigation Summary
At 4:45 p.m. on December 9, 2017, an employee was installing a rigid foam insulation board on the top cupola that was located on the dairy parlor's roof. The employee accessed the metal roof using a bucket truck from the west side of the building and the employee fell approximately 22 feet to the ground on the east side of the building. The employee was transported to Parkview Hospital where he died from fall-related injuries.
