Investigation Summary
At 5:43 p.m. on February 8, 2018, an employee was reaching for a box while working approximately 10 feet up on a Man-Up Order Picker. The employee fell off of the Order Picker and struck the warehouse cement floor. The employee died as a result of multiple blunt force injuries six days later.
