Investigation Summary
At approximately 9:44 p.m. on November 2, 2017, Employee #1 was dismantling the set on a theatrical stage following a performance. Employee #1 was feeding rigging chain into a motor box approximately 12 inches from the edge of the stage. Employee #1 fell 5 feet to the concrete floor below the stage. Another employee contacted emergency services. Employee #1 was transported to a medical center. Employee #1 sustained a fractured skull, fractured collarbone, and 8 fractured ribs. Employee #1 died from the injuries 2 days later.
