Investigation Summary

At 3:36 p.m. on October 16, 2018, an employee was walking around a storage area that was located on a enclosed mezzanine type deck. The employee opened a door and fell through the false ceiling on the other side of the door because the area behind the door was beyond the mezzanine deck and the area was actually just the ceiling for the store room below. There were two sliding bolt latches on the door and the door was not hanging square. Some force was required to open and close the door, to and from the closed position. The investigation revealed that the momentum of the employee pushing the door open caused the employee to lose his balance and fall through the door opening and through the false ceiling. The employee suffered blunt force trauma to his head, resulting in his death. The employee's legs struck some card board boxes during the fall and his head struck the concrete floor.
