Investigation Summary
At 8:30 a.m. on December 7, 2018, a crew arrived at a damaged guardrail location on the eastbound shoulder. As the coworkers worked to prepare the area for the installation of a new guardrail end terminal, the employee and coworker began to unbind the material on the company's tractor trailer parked just past the location of the replacement area. The tractor trailer was tightly parked between the white edge line of the lane and existing guardrailing. The coworker walked around the tractor trailer to approach the foreman to discuss the material on the trailer. At approximately 8:30 a.m., an oncoming tractor trailer headed Eastbound on US-33 encroached into the taper/transition area of the shoulder closure striking traffic control cones and running the rumble strips. As the oncoming tractor trailer veered back into the roadway, the employee was killed when struck by the tractor trailer while unbinding the load from the side of the trailer. The employee was working directly on the white edge line in line with the channeling cones.
