Employee is struck and killed by falling machinery

At 10:30 a.m. on September 19, 2018, an employee was assisting with unloading large machinery from a flatbed truck. As a coworker was using a forklift to unload the machinery from the flatbed truck, the employee was signaling. During unlading, the employee inadvertently stepped between the load and the flatbed truck as it was being lowered. The machinery fell forward and struck the employee, who became pinned to the ground. He suffered sever crushing injuries, resulting in his death.
