Investigation Summary
At 2:43 p.m. on May 24, 2017, an employee was picking up tree limbs when an approximately 150 foot tree uprooted and struck employee on the head. The impact from the blow threw the employee onto the ground. The employee's head struck a sharp rock. Employee was wearing his issued hard hat. The employee was killed by being struck on the head by a falling tree, and then being thrown down onto a rock.
