Investigation Summary
At 6:15 p.m. on May 8, 2018, Employee #1 was showing a coworker how to operate a tube cutting machine in order to cut a 2.5 inch diameter tubing used to build rollers. The employee adjusted the machine speed, and the tube popped out when cut, striking the employee on the left side of his neck. He sustained a deep laceration to his neck, killing him.
