Investigation Summary
At 7:05 a.m. on February 26, 2018, an employee was assisting a coworker to connect a truck and trailer/container. While the coworker backed his truck towards the trailer/container, the employee walked behind the trailer/container to remove its pad lock. The coworker inadvertently struck the employee, pinning him between the truck and the trailer/container. As a result, the employee was killed.
