Employee is thrown from scaffold and is killed

At 3:30 p.m. on August 31, 2018, an employee was working in a 8 foot x 16 foot scaffold platform that was supported by a Lull Rough Terrain Forklift. The employee had the operator lift him up as high as the lift would go so he could take a video recording of the building roof. The employee was thrown from the platform when the forklift tipped over and was killed.
