Investigation Summary
At 3:33 a.m. on June 7, 2019, an employee was riding in a standing position as a passenger in the rear of a Set Up Truck. The employee was holding onto the truck's railing and he let go when his hard hat started to fall off. The employee fell out of the truck, struck his head on the street pavement, and was killed with a skull fracture.
