Investigation Summary
At 10:36 a.m. on Decemember 3, 2018, an employee was inspecting and preparing to grease components on a forklift. The employee was positioned between the vertical mast components when the forklift engaged, crushing him between the components. The employee was killed from the pressure from the mast engaging that ultimately crushed the worker between the components of the forklift mast.
