Investigation Summary
At 11:30 a.m. on September 10, 2018, an employee was drilling a concrete surface to install quick ties for a mobile scaffold. While on the mobile scaffold, the employee moved it. The scaffold became unstable and the employee fell to the ground. He suffered a broken neck and was killed.
