Investigation Summary
At 2:30 p.m. on October 5, 2017, an employee was approaching a personnel basket that was attached to the forks of a rough terrain forklift with a telescoping boom. The personnel basket was positioned against the north side exterior wall of the building, west of the leading edge decking. The top left corner of the guardrail system of the basket platform was level with the metal decking that had been installed. There was a small gap (inches) between the leading edge decking and the personnel basket guardrail. When the employee got within two feet of the baskets guardrail system, the employee stepped on a section of insulation that extended beyond the steel decking that was in place. When the employee stepped on the insulation, he fell to the concrete floor below and was killed.
