Investigation Summary
At 9:00 a.m. on August 17, 2017, an employee was stabilizing a 400 to 500 lb wood header beam while coworkers on the other side of the beam were attempting to lift the beam into position. The employee was unable to hold the beam on the ledge and it fell, striking the employee in the head. The employee was killed from blunt force trauma to his head.
