Investigation Summary
At 9:55 a.m. on December 10, 2018, a crew of three employees were assigned by their superintendent to remove the overhead ceiling tiles and brackets. Two employees worked from Werner 10 feet A-frame ladders while the third employee worked on the ground cleaning the removed tiles. After the employees removed the entire overhead tile, they began to remove the brackets. The employees noticed the overhead 4 ft by 2 ft fluorescent lights were being held in by the bracket. The employees decided to remove the overhead lights before removing the brackets. However, all the power to the building was still up and running. While removing the first light, two wires come in contact and tripped all power in building. Building maintenance was contacted to restore power. While the power was off, the employees removed approximately eight (8) overhead lights. Once power was turned back on, Employee #1 took over for another coworker who did not feel comfortable working on the live wires. Employee #1 climbed the 10 feet A-frame ladder, and placed one foot on the top level of the ladder and one foot on the second level of the ladder to reach the wires. Employee #1 came in contact with live electrical wires and fell backwards off the ladder. Employee #1 died from blunt force trauma to the head.
