Investigation Summary
At 5:30 p.m. on December 6, 2018, an employee was assisting a coworker perform repairs on the coworker's personal motor home outside normal business hours. While the employee was underneath the motor home, which was suspended on jacks, the motor home lurched forward, came off the rear jacks and fatally crushed the employee. He sustained blunt force chest injuries and was killed.
