Investigation Summary
At 8:15 a.m. on December 6, 2018, an employee was making maintenance related adjustments to Turntable #3. The employee was straddling a floor opening approximately 4 feet above a conveyor and other equipment to make adjustments to Turntable #3. The employee fell through the opening and was found downstream under Turntable #1. The employee was killed in the incident.
