Investigation Summary

At approximately 8:00 a.m. on June 30, 2018, Employee #1 and a coworker arrived at a residential property to conduct cleaning tasks prior to the arrival of a new tenant, which they regularly did on a weekly basis. Employee #1 and their coworker began bringing in their equipment that included vacuums, buckets, mops, etc. into the home by way of the front steps. The steps consisted of 7.75 inch risers, 11 inch wide treads, and 36 inch high handrail on both sides. The stair tread depth measured 112 inches at the bottom and tapered towards the top that measured 92 inches. Additionally, the stairs consisted of unpainted lumber for its treads, but its risers and handrails were painted. While Employee #1 continued to bring in the last of the supplies, the coworker started cleaning the property. On one of Employee #1s trips up the stairs, he fell and struck his head on the concrete driveway below. A neighbor, who had witnessed the incident, contacted emergency services and accessed the property to notify the coworker as to what had happened. Employee #1 was transported by air to a medical center for treatment; however, he died on July 1, 2018 due to the injuries.
