Investigation Summary

At 8:15 a.m. on July 19, 2019, Employee #1, a bus driver, was loading a passenger onto a transit bus. The passenger was disabled and was in a wheel chair. The employee was using a Braun Century 2 Series Wheel Chair Lift, Model number NCL917FIBRP-2, to elevate the passenger into the bus. The employee accompanied the wheel chair bound passenger on the personnel lift. She tripped on the lift, fell to the ground, and struck her head on the asphalt. Employee #1 was transferred to the hospital and later flown to a shock trauma unit, where she died from her injuries 12 days later.
