Investigation Summary
At 3:15 p.m. on September 6, 2018, an employee was securing a load of lumber on a flat bed semi-truck while it was still being loaded by a forklift. The employee was struck by a load of lumber that had fallen when the semi-truck was struck by another load that was being loaded by the forklift. The employee was hospitalized with multiple blunt force trauma injuries and later died.
