Investigation Summary
At 10:00 a.m. on December 15, 2017, an employee was climbing a 6 foot ladder and stepped on the second rung from the top before straddling the top of the ladder. The employee fell when the ladder became unbalanced and hit his head on the concrete. The employee was transported to the Fort Worth Hospital where he died from his injuries.
