Investigation Summary
At 2:12 p.m. on November 10, 2017, Employee #1 was checking the rear door seal of a truck stopped at the outbound gate. A driver approached the outbound gate stopped his truck behind the truck already at the outbound gate and failed to keep control of his truck. The truck rolled forward and crushed Employee # 1 between the two trucks and he died on the scene.
