Investigation Summary
At 4:00 p.m. on October 8, 2017, Employee #1 entered a customer's 2008 Jeep Grand Cherokee to move it, and to apply glass cleaner by the use of a hand held spray bottle. At a full-service car wash and express lube facility, Employee #1 entered the truck after the vehicle had completed the mechanical wash process. The weather conditions were sunny and dry. The Jeep stopped at the end of the automated conveyor track. The vehicle was not running. Employee #1 entered to apply window cleaner and move it to the manual finishing area. Employee #1 was working with a coworker at the end of the conveyor. Employee #1 sprayed the front windshield, the driver door window, and passenger windows inside the passenger cabin. When Employee #1 started the vehicle it rapidly accelerated out the end of the building and continued approximately 200 feet across the parking lot and a two lane road. The Jeep curved to the left and rolled over into an approximately 30 foot deep ditch which crushed the cab. Employee #1 died from multiple crushing injuries. Employee #1 received blunt head trauma from the crushing of the cab of the vehicle.
