Investigation Summary
At 8:00 a.m. on October 27, 2018, an employee was tossing two boxes of gutter brushes onto the sky bridge. The employee lost his balance in an aerial lift bucket and fell approximately three stories onto the aerial lift vehicle windshield and to the concrete pavement. The employee was killed. The employee was not wearing any personal fall arrest equipment attached to the aerial lift bucket.
