Investigation Summary
At 8:30 a.m. on May 22, 2021, an employee and coworker walked the sixth floor mechanical shaft to access an extension ladder. The coworker heard something fall and found the employee had fallen approximately fifty feet down the mechanical shaft, received multiple fractures due to blunt force trauma and was killed.
