Investigation Summary
At 8:30 a.m. on February 20, 2018, an employee was riding on a pallet that was being lifted by a forklift. During lifting, the employee fell from the pallet and fractured his left leg. He was taken to a nearby hospital, where he was treated and released on the same day. Approximately one month later (March 19, 2018), the employer was notified by the employee's spouse that he had died.
