Employee Is Killed In Fall From Balcony

At 8:45 a.m. on November 12, 2020, an employee was connecting HVAC tubing to out let ducts outside the guardrails of the fly stage during theatre restoration wor k. The employee leaned a ladder on the top rail of the guardrails on the fly sta ge, and worked from the ladder. The ladder created a fulcrum on the top rail and pivoted forward over the guardrails when the employee climbed up. The employee was thrown off the ladder and fell approximately 30 feet to the performance stag e floor. The employee died from blunt force trauma to head and torso.
