Investigation Summary
At 12:45 p.m. on April 2, 2018, an employee was using a hand truck to deliver boxes. As the employee entered the basement of the building to access its elevator (carriage) a stock person selected the elevator (carriage) call button. A makeshift material lift was in the place of the elevator/carriage, which the employee walked into the elevator shaft, thinking the elevator (carriage) was there, and was crushed by the makeshift material lift. His head was caught in between the top of the hand truck and the bottom of the elevator (carriage), crushing his neck and lower jaw. As a result, the employee was killed.
