Investigation Summary
At 5:00 a.m. on June 20, 2018, Employee #1 was crossing over from the median of Interstate 77 to the shoulder area on the Southbound side, in the basket of a JLG 460SJ aerial lift, Serial Number 0300168669, United Rentals Number 10115531.The basket of the aerial lift was approximately eight feet long, 36 inches wide, and 43 inches high. A tractor trailer coming down the interstate struck the basket of the lift. Employee #1 was thrown out of the basket about 90 feet in the southbound direction. The employee sustained blunt force trauma from the impact of the tractor trailer and was killed. Employee #1 was not wearing a safety harness with lanyard.
