Employee is struck by jig and is killed

On June 7, 2019, an employee and coworker were moving a welding jig that weighed approximately 1,000 lbs. The coworker raised the jib approximately six inches from the ground while the employee leaned his body into the jig. The jig wobbled and slide off of the forks, striking the employee. The employee was crushed and killed.
