Investigation Summary
At 11:20 p.m. on December 18, 2019, an employee climbed up an eight foot step ladder and pulling a 20 lb box with both hands to place it onto the mezzanine when it started to slip and fall. The employee try to grab the box and lost her balance, falling backwards from the ladder and struck her head against the floor, fracturing her skull and was bleeding from her brain. The employee was killed.
