Investigation Summary
At 9:00 a.m. on November 21, 2017, a 30 year old male employee was tearing off a section of flat insulated roofing material when he stepped on a deteriorated section of the metal decking and fell 20 feet to the concrete below and was killed. Other workers were present within 30 feet of the employee performing the same type of work and witnessed the incident. Weather was not a factor and the employee was not wearing fall protection.
