Investigation Summary
At approximately 10:00 a.m. on September 21, 2017, Employee #1 was throwing old shingles attached to the plywood of a roof into a dumpster located on the ground. As he threw the shingles down towards the dumpster, he lost his balance and fell 3 ft off the roof. He struck the ground and suffered head and neck injuries that killed him.
