Investigation Summary
At 10:45 a.m. at June 2, 2017, a coworker was taking a break. Once the coworker returned from from his break, he heard the employee struggling to breathe and later found the employee on the ground. The employee fractured his back and was bleeding from his head. The employee was killed.
